Sir Charles Gairdner Hospital has issued an apology to the family of a Perth grandmother who say she's endured what they believe to be neglectful care in multiple Perth hospitals.

A Perth hospital has apologised to the family of a grandmother whose face was left a “complete mess” by “avoidable” falls at the major health facility.

Marianne Molloy, 68, suffered a broken nose and significant facial bruising during what her loved ones say was neglectful care at Sir Charles Gairdner over the last week.

It comes after Molloy was allegedly assaulted by a hospital worker at Royal Perth Hospital (RPH) in June, with that matter now before the courts.

The grandmother was taken to Sir Charles Gairdner Hospital after a fall and is on a one-to-one assist ratio, meaning staff are supposed to be at her side every time she leaves her bed.

But her family says that did not happen, and she was seriously hurt when she should have been protected.

“When I went in (on Sunday morning), after she had had another fall, there was no signage to say she’s a high falls risk,” daughter Veronica Molloy told 7NEWS.

“The (assistant in nursing) that I spoke to had absolutely no handover and was unaware that mum was a one assist and that’s why she was left in the toilet while she tended to another patient.”

The 68-year-old has a condition similar to dementia and fell twice in as many days.

On one occasion, her nose was smashed on the bathroom floor.

“Her face is a complete mess, it’s heartbreaking,” Molloy told 7NEWS.

Loved ones “don’t know where to turn, and who to ask for help”, saying they want to prevent “somebody else going through what mum’s going through”.

“She is afraid. I needed to go home to get some sleep and she was begging me to stay. That was hard,” Molloy said.

Sir Charles Gairdner Hospital has issued an apology to the family, saying it focuses on providing high-quality care and will investigate the falls.

The alleged assault at RPH is currently before the courts.

It is understood the accused hospital worker is not an RPH staff member and was hired by an agency.

That man is due to face court at the end of October.
