Major Crash investigators are examining the incident to work out exactly what went wrong and have appealed for information from witnesses.

The tragedy was one of four fatal incidents on WA roads during a shocking weekend that took the annual road toll to a level not experienced since 2016.

A 21-year-old motorbike rider was killed when his bike left the road and struck a tree in Gidgegannup, 40km northeast of Perth, about 10.10am on Saturday.

A 16-year-old pillion passenger was rushed to hospital with critical injuries following the Clenton Rd crash.

A road train with three trailers hit a 40-year-old female pedestrian on Prinsep St in Norseman, in WA’s Goldfields-Esperance region, about 11.15am on Saturday.

About 15 minutes later — in Carlisle, Perth — a 77-year-old woman was killed when a silver Hyundai i30 collided with a white Mitsubishi Challenger on Oats St.

The woman was rushed to Royal Perth Hospital with critical injuries but could not be saved.

The 26-year-old Mitsubishi driver was not injured.
