Friends in a second vehicle noticed the SUV was no longer following and returned to find the crash scene, along with a local property owner.
The group managed to rescue a woman in her 20s from the vehicle, but efforts to free the remaining two occupants, believed to be a man and a second woman, were unsuccessful.
The injured woman was airlifted to John Hunter Hospital in serious but stable condition.
The multi-agency rescue operation is set to continue throughout the morning, with Morialta Road remaining closed in both directions.
